Private (Non‑NHS) Fees – Important Information for Patients

Some services requested by patients are not covered by the NHS, such as private letters, reports, insurance forms, and certain medical assessments. As these services fall outside NHS care, a fee applies.

GPs are not obliged to carry out non‑NHS work. Any private work is completed at the GP’s discretion and in their own time, alongside their NHS responsibilities. For this reason, not all requests can be accepted.

Our administration team will advise whether your request can be undertaken and confirm the applicable fee. A full list of current fees is provided below.

Payment information

  • All fees must be paid on collection
  • Valid photo ID is required
  • Payment can be made by cash or cheque, payable to Chertsey Health Centre
  • We regret that card payments are not available

Medical Certificates

Officially called MED3 certificates, but most commonly known as ‘sick notes’. These are ONLY issues for an illness lasting 7 days or more, and ONLY for the purposes of obtaining sick payment benefits.

For sickness lasting less than 7 days, we will not provide a sick note. If your employer insists then there will be a charge for this.

Applications for a Firearms / Shotgun License

The practice does not complete the medical proforma in relation to any applications for a firearms / shotgun license or a renewal of a license.

For completion of the medical proforma patients will need to make arrangements with another private medical practitioner; and as that practitioner will need to see their medical records, it is recommended patients sign up to our online access to your medical records.

Stopping Smoking

OneyouSurrey can offer smoking cessation support and advice.

Travel Vaccinations

We only offer NHS Travel Vaccinations only and these need to be pre-booked via our website. As we have limited availability the more notice you can give us the more we can accommodate your request.

Well Woman Clinics

This service is provided by our practice nurses. We strongly urge all women patients to attend for routine cervical smears when asked to do so.

Warfarin Monitoring

These clinics are booked by the INR department at St. Peter’s hospital, please contact 01932 87200.
