Chertsey Health Centre Registering Families Together Guidance

Statement

  • The practice will normally register and allocate family members (defined as individuals living in the same household or immediate relatives) under the same GP.
  • This approach is intended to:
  • Provide continuity of care across family members.
  • Support holistic understanding of family health needs.
  • Simplify communication and administration.

Exceptions

  • Patients retain the right to request a different GP if they prefer.
  • If a relationship between a patient and their allocated GP breaks down, the patient may be reassigned to another GP.
  • In such cases, the practice will normally reallocate the entire family unit to the new GP to maintain consistency, unless family members specifically request otherwise.
  • Clinical need, workload distribution, and patient preference will all be considered when making allocation decisions.
