In order to provide you with appropriate medical care, we need to hold and process personal and medical information about you. This information is stored securely on our computer systems and in paper records.

Please help us to keep your records accurate and up to date by informing the practice of any changes to your personal details or circumstances.

Access to your medical records

Doctors and authorised members of the practice team may access your medical records only where this is necessary to carry out their duties. Your GP is responsible for the accuracy, confidentiality, and safe‑keeping of your records.

From time to time, it may be necessary to share relevant information with other healthcare professionals involved in your care. Anyone who has access to your records is appropriately trained in confidentiality and is bound by both legal and contractual obligations to protect your privacy.

Data security

All information about you is held securely, and appropriate safeguards are in place to prevent unauthorised access, accidental loss, or misuse of your data.

Disclosure of information

In certain circumstances, we may be legally required to disclose information to statutory or official bodies, for example:

  • Where a court order is presented
  • For public health or safeguarding reasons

In other situations, such as requests for insurance reports, employer reports, or legal documents, your written consent will be required before any information is released.

Your privacy

To protect your confidentiality:

  • We will not disclose personal or clinical information over the telephone unless we are satisfied that we are speaking directly to you
  • Information will not be shared with family members, friends, or spouses without your prior written consent
  • We do not leave messages or share information with third parties on your behalf
